I hereby authorize the University Prep to have a doctor provide emergency treatment for my student in the event that I cannot be contacted, and I agree to assume financial responsibility for this service.

Realizing that there is risk inherent in all and any activities and events including outdoor recreational activities and sporting events and in consideration of my student being allowed to participate in all and any activities and events including outdoor recreational activities and sporting events, I agree to release, indemnify, and hold harmless University Prep, together with its trustees, faculty, volunteers, trustees, and other agents, from any and all claims, liabilities, and damages relating to any injury, sickness, death or destruction of any property which may arise out of or result from or be in any way connected with the participation of my student in the UPrep Camp, other than claims, liabilities or damages based on the gross negligence of University Prep or its faculty.

In consideration for the student's being permitted to participate in the UPrep Camp,the undersigned hereby release, waive and forever discharge from any liability and hold harmless and indemnify the School, its officers, directors, trustees, faculty, instructors, coaches and agents, for any injury to the student arising out of or resulting from the student's participation in the UPrep Camp, but only to the extent that such injury is not caused by the gross negligence or willful misconduct of the Indemnified Party, and for any damages and costs, including attorney's fees and cost of litigation which may be incurred by any Indemnified Party in defense of a claim or claims brought against them by the student or any other person or entity.

This Waiver and Release of Liability shall be legally binding upon the student, the students' parents or guardians, heirs, personal representatives, and assigns. The undersigned have carefully read this Waiver and Release of Liability and fully understand its contents. The undersigned are aware that they are releasing certain legal rights by this release that the undersigned otherwise may have. The undersigned sign this Waiver and Release of Liability of each of the undersigned's free will.

I the parent/guardian, acknowledge that I am voluntarily requesting that my student participate in The University Prep Basketball Summer Camp (UPrep Camp) The undersigned hereby gives consent for the student to participate in the Program, provided that the undersigned may withdraw consent for the student's participation in a particular Program by providing written notice to the Assistant Athletic Director at abrooks@univesityprep.org to be received by him within a reasonable time prior to such UPrep Camp.

The undersigned understand that the UPrep Camp may expose the student to risks of injury.

I understand that all sports and other recreational activities involve a certain amount of risk. I hereby give permission for my child/ward to participate in the UPrep Camp offered by University Prep and I assume and accept all risk related to such participation.

I agree that University Prep shall not be liable for any injuries or harm to my student, or to my student’s property, or be subject to any claim, demand, injury, or damages whatsoever arising out of my student’s participation in the UPrep Camp. I also understand that University Prep cannot eliminate or prevent injuries from occurring to participants.

Further, my student has no medical condition which would prohibit him from participation in physical activities.

I hereby give permission that in the event of any illness or injury, my student may be treated for such illness or injury.
